Many Christians treat marriage as a covenant. An influential group of contemporary Christians argues that covenant marriage provides a response to what they regard as the social ills of high divorce rates and the ‘breakdown’ of the traditional family. These Christians often look to John Calvin's marriage theology for inspiration because he linked treating marriage as a covenant to regarding marriage as sacred and indissoluble. In this article I cast doubt on the wisdom of treating marriage as a covenant. I (...) develop a critical genealogy that shows Calvin's marriage theology oppressed women by reinforcing their subordination to men and trapping women in abusive marriages. I then argue that recent social science suggests covenant marriage functions similarly among contemporary Christians. I consider whether a non-oppressive form of covenant marriage can be developed, before suggesting that we cannot yet know whether covenant marriage is salvageable. (shrink)

Employing the National Institute of Mental Health-funded Prevention of Suicide in Primary Care Elderly Collaborative Trial as a case study, we discuss 2 sets of ethical issues: obtaining informed consent for a clinic-based intervention study and using treatment as usual (TAU) as the control condition. We then address these ethical issues in the context of the debate about the quality improvement efforts of health care organizations. Our analysis reveals the tension between ethics and scientific integrity involved with using TAU as (...) a control condition and the difficulty in designing high-quality research in a community-based setting. The phenomenology of auditory verbal hallucinations occurring in hypnagogic and hypnopompic states has received little attention. In a sample of healthy participants, 108 participants reported H&H AVHs and answered subsequent questions on their phenomenology. AVHs in the H&H state were found to be more likely to only feature the occasional clear word than to be clear, to be more likely to be one-off voices than to be recurrent voices, to be more likely to be voices of people known to the (...) individual than unknown persons, to be more likely to talk directly to the person rather than not, and to only rarely give commands, ask questions, or to result in an interactive conversation. Their phenomenology was similar to normative AVHs in wakefulness in that the voice-hearer was usually the target of the voice, and the voice was more likely to be of a recognized person. However, H&H AVHs differed from AVHs in wakefulness in that commands and questions were rare, and there was typically no dialogical engagement with the voice. We conclude by proposing that two distinct types of H&H AVHs may exist, based on an analysis of the phenomenology of the experience, and suggest avenues for future research. (shrink)

A geographically stratified cross-sectional survey was conducted in 2002 to investigate household-level factors associated with use of mosquito control measures and self-reported malaria in Malindi, Kenya. A total of 629 households were surveyed. Logistic regressions were used to analyse the data. Half of all households (51%) reported all occupants using an insecticide-treated bed net and at least one additional mosquito control measure such as insecticides or removal of standing water. Forty-nine per cent reported a history of malaria in the household. (...) Of the thirteen household factors analysed, low (OR=0·23, CI 0·11, 0·48) and medium (OR=0·50, CI 0·29, 0·86) education, mudcoral (OR=0·0·39, CI 0·24, 0·66) and mud block–plaster (OR=0·47, CI 0·25, 0·87) wall types, farming (OR=1·38, CI 1·01, 1·90) and travel to rural areas (OR=0·48, CI 0·26, 0·91) were significantly associated with the use of mosquito control, while controlling for other covariates in the model. History of reported malaria was not associated with the use of mosquito control (OR=1·22, CI 0·79, 1·88). Of the thirteen covariates analysed in the second model, only two household factors were associated with history of malaria: being located in the well-drained stratum (OR=0·49, CI 0·26, 0·96) and being bitten while in the house (OR=1·22, CI 0·19, 0·49). These results suggest that high socioeconomic status is associated with increased household-level mosquito control use, although household-level control may not be enough, as many people are exposed to biting mosquitoes while away from the house and in areas that are more likely to harbour mosquitoes. Introduction Comparative psychology can generally be defined as the branch of psychology that studies the similarities and differences in the behavior of organisms. Formal definitions found in textbooks and encyclopedias disagree whether comparative psychologists restrict their work to the study of animals or include the study of human behavior. This paper offers an opinion on the major problem facing comparative psychology today – where we will find the next generation of comparative psychology students. Something must be done before we lose (...) access to an entire generation of psychology undergraduates. It is not enough to say that comparative psychology is being “absorbed” in other academic units such as “integrative biology”, “integrative study of animal behavior”, “evolutionary psychology”, “comparative cognition”, or “animal cognition.” Indeed, many of these topics are not primarily studied in psychology departments, wherein comparative psychology students have traditionally received joint training in the principles of behavior and comparative analysis, as applied to both humans and other animals. Consider, for example, the difference between comparative psychology and comparative cognition. Comparative cognition includes features of comparative psychology, but they are not identical. This symposium examines insurrectionist ethics, the brainchild of Leonard Harris. The position does not stem from one key source; it was born out of Harris’s philosophical interaction with various philosophers over an extended period, including thinkers as diverse as David Walker, Karl Marx, Edward Wilmot Blyden, Alain Locke, and Angela Davis. The driving questions are: What counts as justified protest? Do slaves have a moral duty to insurrect? What character traits and modes to resistance are most conducive to liberation and (...) the amelioration of oppressive material conditions? Insurrectionist ethics is meant to address such questions. The American philosophical tradition is often portrayed as a genteel tradition that is committed to democracy and the incremental expansion of democracy through suasionist means. In an attempt to complicate this narrative, the author articulates the basic features of Leonard Harris’s insurrectionist ethics, then attempts to locate this insurrectionist ethics in the work of Henry D. Thoreau. It is argued that this insurrectionist ethos is a fecund addition to the American philosophical tradition and that insurrectionist character traits and modes of (...) comportment offer appealing resources to those faced with oppression and hegemony. In July 2011, the ACGME implemented new rules that limit interns to 16 hours of work in a row, but continue to allow 2nd-year and higher resident physicians to work for up to 28 consecutive hours. Whether the ACGME's 2011 work hour limits went too far or did not go far enough has been hotly debated. In this article, we do not seek to re-open the debate about whether these standards get matters exactly right. Instead, we wish to address the (...) issue of effective enforcement. That is, now that new work hour limits have been established, and given that the ACGME has been unable to enforce work hour limits effectively on its own, what is the best way to make sure the new limits are followed in order to reduce harm to residents, patients, and others due to sleep-deprived residents? We focus on three possible national approaches to the problem, one rooted in funding, one rooted in disclosure, and one rooted in tort law. (shrink)

Over the past decade, a series of studies have found that physicians-in-training who work extended shifts are at increased risk of experiencing motor vehicle crashes, needlestick injuries, and medical errors. In response to public concerns and a request from Congress, the Institute of Medicine conducted an inquiry into the issue and concluded in 2009 that resident physicians should not work for more than 16 consecutive hours without sleep. They further recommended that the Centers for Medicare & Medicaid Services and the (...) Joint Commission work with the Accreditation Council for Graduate Medical Education to ensure effective enforcement of new work hour standards.The IOM's concerns with enforcement stem from well-documented non-compliance with the ACGME's 2003 work hour rules, and the ACGME's history of non-enforcement. In a nationwide cohort study, 84% of interns were found to violate the ACGME's 2003 standards in the year following their introduction. (shrink)
